Mesothelioma is a rare and aggressive cancer that forms in the mesothelium, a thin protective membrane that lines the chest cavity, abdominal organs, heart, and testes. The mesothelium produces a small amount of lubricating fluid that allows these organs to move smoothly against each other during breathing, digestion, and other normal functions. When cancer develops in this tissue, it disrupts organ function and spreads along the membrane surfaces in patterns that make it difficult to treat with standard approaches.
There are different types of mesothelioma depending on where the tumors form, but they all share one overwhelming cause: exposure to asbestos.
Asbestos fibers are microscopic, sharp, and nearly indestructible. When inhaled or swallowed, they can become lodged in the mesothelium, where they remain for the rest of a person's life. Over time, those fibers trigger chronic inflammation, cellular damage, and the accumulation of genetic mutations that eventually produce cancer. The entire process from first exposure to mesothelioma diagnosis typically spans 20 to 50 years or more. That's why mesothelioma is being diagnosed today in people whose exposure happened decades ago on job sites that no longer exist, with products that have long since been discontinued, made by companies that declared bankruptcy decades ago.
Mesothelioma was virtually nonexistent before asbestos was widely used in American industry. In a review of 2,276 consecutive autopsies performed at New York's Mount Sinai Hospital between 1883 and 1910 (before commercial asbestos use took off) not a single case of mesothelioma was identified. The disease simply did not exist in the population because the population was not being exposed to asbestos. That changed dramatically in the twentieth century, as asbestos was added to thousands of products and millions of workers were exposed without warning. As journalist Paul Brodeur documented in 1968: asbestos had become "practically ubiquitous in modern society," present in every automobile, airplane, ship, factory, home, and farm across the country.
Today, approximately 3,000 to 4,000 Americans are diagnosed with mesothelioma each year, and that number has remained largely stable for decades despite reduced asbestos use. The long latency period means the consequences of mid-twentieth-century exposure are still arriving.
Mesothelioma is not a random disease caused by genetics, lifestyle, or bad luck. It is overwhelmingly the result of asbestos exposure.
Types of Mesothelioma by Location
Mesothelioma is classified first by where in the body it develops. The location determines the symptoms a patient experiences, the diagnostic process, the treatment options available, and the expected prognosis. Each type is named for the specific mesothelial membrane where the cancer originates.
Pleural Mesothelioma (Lining of the Lungs)
Pleural mesothelioma is the most common form, accounting for 75 to 90% of all mesothelioma diagnoses. It develops in the pleura, the two-layered membrane that surrounds the lungs and lines the inside of the chest wall. Peritoneal Mesothelioma (Lining of the Abdomen)

Pericardial Mesothelioma (Lining of the Heart)
Pericardial mesothelioma is extremely rare, accounting for less than 1% of all mesothelioma cases. It develops in the pericardium, the membrane surrounding the heart. What Causes Mesothelioma
Mesothelioma is one of the rarest cancers in the world, yet it has one of the clearest and most thoroughly documented causes of any human malignancy. The causal link between asbestos and mesothelioma is not a matter of scientific debate. It is the settled consensus of every major public health, regulatory, and research organization in the world:
The World Health Organization (WHO) states unequivocally that "all forms of asbestos, including chrysotile, are carcinogenic to humans" and estimates that occupational asbestos exposure causes more than 200,000 deaths per year worldwide.
The National Cancer Institute (NCI) confirms that asbestos has been classified as a known human carcinogen by the U.S. Department of Health and Human Services, the Environmental Protection Agency, and the International Agency for Research on Cancer — and that "it is thought that most mesotheliomas are due to asbestos exposure."
The Agency for Toxic Substances and Disease Registry (ATSDR), a division of the Centers for Disease Control and Prevention, states that "all forms of asbestos are hazardous, and all can cause cancer."
The National Institute for Occupational Safety and Health (NIOSH) has issued formal intelligence bulletins documenting the carcinogenicity of asbestos in occupational settings and recommending that workplace exposures be reduced to the lowest feasible concentration.
The U.S. Environmental Protection Agency (EPA) determined in 2020 that chrysotile asbestos poses unreasonable risks to human health and finalized a rule in March 2024 banning ongoing uses of chrysotile asbestos in the United States. The agency cited the need to "protect people from lung cancer, mesothelioma, ovarian cancer, laryngeal cancer and other health problems caused by asbestos exposure."
The Occupational Safety and Health Administration (OSHA) regulates asbestos as a known workplace carcinogen, setting permissible exposure limits and requiring employers to monitor, control, and remediate asbestos hazards across all industries where exposure occurs.
There is no established safe level of exposure to asbestos. There is no threshold below which asbestos contact is harmless. In epidemiology, mesothelioma is considered a signal tumor for asbestos exposure: the disease signals that a person was exposed to asbestos.
The Discovery That Changed Everything
The causal link between asbestos and mesothelioma was not always known. Before 1960, mesothelioma was considered an extremely rare tumor, so uncommon that it was barely recognized in medical literature. The breakthrough came in 1960, when South African pathologist Dr. J.C. Wagner published a landmark study in the British Journal of Industrial Medicine documenting an epidemic of mesothelioma among residents of the Cape Province, near crocidolite (blue asbestos) mines.
Wagner's study analyzed 33 cases of pleural mesothelioma — a remarkable number of cases of what had previously been an almost unknown disease. What made Wagner's findings extraordinary was not just the number of cases but who was getting sick. Only 8 of the 33 patients had evidence of occupational exposure to asbestos, while 20 of the remaining 25 had lived near the mines as infants. Many of those diagnosed had never worked directly with asbestos but were family members, neighbors, and children of asbestos workers.
Wagner demonstrated that 32 of the 33 patients had some form of contact with asbestos, establishing strong circumstantial evidence for the causal relationship. For the first time, scientists recognized that brief, low-level, non-occupational contact with asbestos could cause a fatal cancer. Wagner's study revealed that mesothelioma was both an occupational hazard for asbestos workers and a community-wide health threat to those who lived near asbestos facilities.
Wagner's study ignited a wave of international research. Within a few years, physicians at Mount Sinai Hospital in New York and medical centers across Europe were documenting the disease in insulation workers, shipyard laborers, and factory hands. They also found the disease in wives and children of asbestos workers who had never set foot in a workplace but had been exposed to fibers carried home on clothing, skin, and hair. The global medical consensus began to take shape: asbestos exposure caused mesothelioma, and no amount of exposure was safe.
What the Asbestos Industry Knew — and When They Knew It
The medical community's understanding of asbestos and mesothelioma was built through rigorous, independent research. The asbestos industry's understanding was built through something very different: their own internal studies, conducted decades earlier, that they systematically suppressed.
As early as the 1930s, asbestos manufacturers had medical evidence that their products were causing fatal lung disease. By the 1940s and 1950s, they understood the connection between asbestos and cancer. But rather than warn their workers or the public, they made a calculated business decision to conceal the evidence. They classified internal studies. They pressured researchers to alter findings. And they funded public relations campaigns emphasizing the "safety" of asbestos products while workers were dying.
They got away with it for decades because the disease didn't appear until 20 to 50 years after exposure. By the time workers started getting diagnosed, the connection to their exposure was obscured by the passage of time. The companies that had exposed them had moved on to new products, new markets, and new workers.
It took a dying insulation worker from Texas to change that. In 1973, Clarence Borel became the first person in American history to win a lawsuit proving that asbestos had caused his mesothelioma and that the companies who manufactured the products he used were legally responsible. Borel filed his case because he knew the evidence in his lawsuit could expose the cover-up and protect others who were still being exposed. He was right. The Borel decision opened the floodgates of litigation that eventually forced the asbestos industry's internal documents into the public record. And that finally revealed a decades-long corporate conspiracy that had knowingly traded human lives for profit.

The Latency Period of Mesothelioma
One of the defining characteristics of mesothelioma is the extraordinary length of time between first asbestos exposure and the development of symptoms. In most cases, the latency period is 20 to 50 years, though cases with latency exceeding 60 or even 70 years have been documented. This means a construction worker exposed to asbestos insulation in 1970 might not develop symptoms until 2020 or later. A Navy veteran who served on asbestos-insulated ships in the 1960s might not be diagnosed until he is in his seventies or eighties.
The latency period explains why mesothelioma incidence has remained stable at approximately 3,000 to 4,000 new cases per year in the United States despite significantly reduced asbestos use since the 1980s. The exposures that are producing today's diagnoses happened decades ago, and the pipeline of latent disease will continue to generate new cases for years to come. It also explains why asbestos companies were able to conceal the danger for so long.
Other Rare Causes of Mesothelioma
Asbestos causes the overwhelming majority of mesothelioma cases, but a small number of cases involve other contributing factors:
Genetic susceptibility. Mutations in the BAP1 gene can predispose individuals to mesothelioma by impairing the body's ability to repair DNA damage. Families with inherited BAP1 mutations have elevated rates of mesothelioma and certain other cancers. However, a BAP1 mutation alone is almost never sufficient to cause mesothelioma. In nearly every documented case, asbestos exposure triggered the disease. The genetic vulnerability explains why some individuals develop mesothelioma after relatively modest exposure while others with heavier exposure do not, but the asbestos remains the proximate cause. Even people who are genetically more susceptible to mesothelioma can seek compensation when asbestos exposure triggers the disease. That principle is taught to first-year law students as the eggshell skull doctrine: if your negligence causes an injury in a person whose skull is unusually fragile, the person's fragility is not a defense. Put another way: "You broke it, you bought it." Companies that exposed a genetically vulnerable person to asbestos are fully liable for the resulting cancer. The victim's predisposition does not reduce their responsibility — it simply explains why the damage their negligence caused was so severe.
Therapeutic radiation. High-dose radiation therapy delivered to the chest or abdomen for treatment of other cancers (such as Hodgkin's lymphoma) has been identified as an independent cause of mesothelioma in a small number of cases. Radiation-induced mesothelioma tends to have a shorter latency period than asbestos-related disease, and it develops in the radiation field rather than following the anatomical distribution associated with fiber exposure.
Childhood mesothelioma. In rare cases, children develop mesothelioma without a clear exposure history. Some of these cases are associated with BAP1 or other inherited mutations. Others remain unexplained, and illustrate the genuine gaps in scientific understanding that the research community continues to investigate.
Idiopathic mesothelioma. "Idiopathic" is the medical term for a disease with no identified cause. A small percentage of mesothelioma cases are classified as idiopathic because no asbestos exposure history can be documented. But that label deserves scrutiny, because the absence of a documented exposure is not the same as the absence of an actual exposure. Self-reporting of asbestos contact is notoriously unreliable. In one study, researchers used analytical transmission electron microscopy to examine lung tissue from mesothelioma patients who reported no history of asbestos exposure — and found that 61% of patients had in fact been exposed to large quantities of asbestos. They simply didn't know they had been exposed.
Many cases labeled "idiopathic" are almost certainly cases in which a more thorough investigation would reveal the exposure that caused the disease. And critically, if asbestos exposure of any kind is eventually identified, the case is no longer idiopathic by definition. Because there is no safe level of asbestos exposure, any confirmed contact is sufficient to establish causation. A mesothelioma that develops in a person who was exposed to asbestos is an asbestos-caused mesothelioma — regardless of how brief or seemingly minor the exposure was.
Debunking the SV40 Theory of Mesothelioma Causation
Simian Virus 40 (SV40), a monkey virus that contaminated early polio vaccines administered between 1955 and 1963, was once proposed as a potential cause of mesothelioma. Early laboratory studies showed SV40 could transform human cells in vitro, and some researchers reported detecting traces of the virus in mesothelioma tumor samples, leading to speculation that the contaminated vaccines might have contributed to mesothelioma risk in people who received them.
That theory has not held up. Multiple large-scale epidemiological studies comparing cancer rates in people who received SV40-contaminated vaccines against those who received uncontaminated vaccines found no increased risk of mesothelioma or any other cancer in the contaminated group. Further investigation revealed that many of the original positive detections of SV40 in tumor samples were attributable to laboratory contamination and flawed testing methodology, not to genuine viral presence in human tissue. The medical consensus today is unambiguous: SV40 is not a credible cause of mesothelioma.
The theory is worth mentioning here because, despite being scientifically discredited, some defense attorneys still raise SV40 in asbestos litigation in an effort to suggest that their client's product was not the cause of a plaintiff's cancer. It is the same strategy the asbestos industry has always used: introduce doubt where the science is clear, and hope that complexity creates confusion. The epidemiological evidence does not support it.
If someone has mesothelioma and a history of asbestos exposure — even 50 or more years ago — asbestos is almost certainly the cause. That is the settled consensus of the international medical community.
How Mesothelioma Is Diagnosed
Mesothelioma can be difficult to diagnose, particularly in its early stages, because its symptoms overlap with far more common conditions. Pleural mesothelioma mimics pneumonia, COPD, and lung cancer. Peritoneal mesothelioma is mistaken for irritable bowel syndrome, ovarian cancer, or liver disease. Pericardial mesothelioma resembles congestive heart failure. And testicular mesothelioma is almost always treated as a benign hydrocele before the cancer is discovered. The result is that many patients experience months of evaluation for the wrong condition before the correct diagnosis is reached.
Accurate diagnosis requires a combination of imaging, fluid testing, tissue biopsy, and specialized pathological analysis. Each of these steps plays a different role, and for most patients, tissue biopsy is the only way to reach a definitive diagnosis.
Diagnostic Imaging
Diagnosis often begins with imaging ordered to investigate unexplained symptoms. A contrast-enhanced CT scan is the standard first-line study for both pleural and peritoneal mesothelioma, capable of identifying pleural thickening, effusions, peritoneal nodules, ascites, and suspicious masses. CT provides a preliminary picture of disease extent but cannot confirm the diagnosis on its own. That's because the findings it produces that are consistent with mesothelioma are also consistent with other conditions.
PET-CT combines anatomical and metabolic imaging to identify areas of active tumor growth and can detect metastatic disease that CT alone may miss. MRI provides superior soft-tissue detail and is particularly valuable for evaluating tumor invasion into the diaphragm, chest wall, or bowel surfaces. Ultrasound is often used to guide diagnostic procedures such as thoracentesis or paracentesis and, in testicular mesothelioma, is typically the first imaging study performed to evaluate scrotal swelling.
No combination of imaging studies can definitively diagnose mesothelioma. Imaging identifies where to look for tissue to biopsy, but the biopsy is what confirms the diagnosis.
Fluid Testing
In many mesothelioma cases, fluid collects in the body cavity where the tumor is growing: pleural effusion in the chest, ascites in the abdomen, pericardial effusion around the heart, or hydrocele fluid around the testicle. Draining this fluid through thoracentesis, paracentesis, pericardiocentesis, or aspiration serves both therapeutic and diagnostic purposes because it relieves symptoms and provides a sample that can be examined for cancer cells.
However, fluid cytology detects mesothelioma in fewer than 30% of cases. The malignant cells present in fluid are often difficult to distinguish from benign reactive mesothelial cells and from the cells of other cancers. Fluid analysis is useful as a preliminary step but is almost never sufficient to confirm a mesothelioma diagnosis.
Biopsy: The Gold Standard for Mesothelioma Diagnosis
A tissue biopsy is required for a definitive mesothelioma diagnosis. The specific approach depends on the type and location of disease.
For pleural mesothelioma, video-assisted thoracoscopic surgery (VATS) is the preferred method, allowing the surgeon to directly visualize the pleural cavity, obtain multiple tissue samples, and in some cases perform therapeutic procedures during the same session. VATS provides diagnostic sensitivity approaching 95% or higher. For peritoneal mesothelioma, diagnostic laparoscopy serves a dual function: it confirms the diagnosis through targeted biopsies and simultaneously assesses the Peritoneal Cancer Index, which determines surgical candidacy for CRS-HIPEC. CT-guided needle biopsy is an alternative when minimally invasive surgery is not feasible, though it yields smaller samples that may limit the pathologist's ability to determine the histological subtype. For testicular mesothelioma, diagnosis is typically made on the tissue obtained during orchiectomy.
Pathology and Immunohistochemistry
Once tissue is obtained, a pathologist examines it under a microscope and performs immunohistochemical (IHC) staining to confirm the diagnosis and distinguish mesothelioma from other cancers that can mimic it. Mesothelioma tumors are typically positive for calretinin, WT-1, cytokeratin 5/6, and D2-40 (markers of mesothelial origin) and negative for CEA, MOC-31, BerEP4, and TTF-1 (markers associated with adenocarcinomas and other tumors). This staining panel is what separates a confirmed mesothelioma diagnosis from a suspected one.
The pathology report will also identify the histological subtype as epithelioid, sarcomatoid, or biphasic, which is essential for every subsequent treatment decision.

Misdiagnosis has real consequences: different cancers require different treatments, and a patient treated for the wrong disease is losing time they cannot afford to lose. Outcomes at high-volume specialized centers consistently and measurably outperform those at community hospitals treating the same disease. This has been documented directly in peritoneal mesothelioma, where treatment at an academic center doubles the median survival time, and the same pattern holds across all forms of the disease. The rarity of mesothelioma means that the institutional experience, surgical volume, multidisciplinary tumor boards, and clinical trial access that drive better outcomes are concentrated at a small number of centers.
Pleural Mesothelioma
Treatment for pleural mesothelioma may include pleurectomy/decortication (P/D) or extrapleural pneumonectomy (EPP). Both are surgical approaches that remove the diseased pleural tissue, with EPP also removing the lung on the affected side. First-line chemotherapy is typically cisplatin or carboplatin combined with pemetrexed. The FDA approval of nivolumab plus ipilimumab (checkpoint inhibitors) for unresectable pleural mesothelioma marked a landmark advance in systemic treatment, and multiple clinical trials are currently evaluating additional immunotherapy combinations, targeted therapies, and novel drug delivery methods.

Peritoneal Mesothelioma
For eligible patients, the standard treatment is cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (CRS-HIPEC). That's a procedure that surgically removes all visible tumor from the abdominal cavity and then bathes the peritoneal surfaces with heated chemotherapy delivered directly to the site of disease. CRS-HIPEC has transformed outcomes in peritoneal mesothelioma, extending median survival from approximately one year to three years or more at experienced centers, with some patients surviving five to ten years after treatment. Pericardial Mesothelioma

Radical inguinal orchiectomy is the standard first-line surgical treatment for testicular mesothelioma. Increasingly, aggressive approaches that include hemiscrototomy and retroperitoneal lymph node dissection (RPLND) are being used at specialized centers. The 2025 Memorial Sloan Kettering series demonstrated that a multimodal protocol combining aggressive surgery with selective systemic therapy produced a median survival of 4.5 years — roughly double the historical two-year benchmark. Chemotherapy is extrapolated from pleural mesothelioma protocols, and immunotherapy with checkpoint inhibitors is emerging as an option for recurrent or metastatic disease.
